

Incidence Reporting Digital Forms and Management
The creation of online and offline digital forms, including incidence reporting, is made easy using checkboxes, drop down menus, likert scales, text fields, and logic rules. These help to dynamically reveal form fields based on answers provided or new add fields to account. When completing forms, respondents can choose from a well linked database system. All data is saved into OOHMS management module. Two dashboard interfaces exist for the admin and user (employee).
